Welcome to the Driver's page!

If you are interested in applying for a position as a Class A CDL driver with Laughing Horse Enterprises please follow the following steps:

To read and print forms, you must have the Adobe Acrobat Reader. Click on the icon to get it.

1.
The first step is to obtain a recent copy of your motor vehicle driver's record (abstract).
Please fax a current copy of your motor vehicle driver's record to 509-635-0102.

If you do not have a current copy of your motor vehicle driver's record we can obtain one for you (we cannot obtain driver records from the following states: NY, NJ, NH).
Please print out the MVR form below, complete and sign, fax back to 509-635-1356:
Motor Vehicle Record Request Form

Please fax the following documents along with your motor vehicle driver's record or MVR request form:
- an enlarged black & white copy of your commercial driver's license
- an enlarged copy of your Medical Examiner's Certificate
- an enlarged copy of your Social Security Card

If our safety department determines that your motor vehicle driver's record is satisfactory, you can move on to the next step.

2.
The second step is to print out the following forms, complete and sign each one, and return originals in the mail to:

Drug and Alcohol Testing Program please fill out and fax back pages 4, 5, 6, & 9 (only fax the pages you sign, no need to fax or send all the pages)
Motor Vehicle Driver's Certification of Violations
Release of Information Form
Release and Documentation of Pre-employment Testing Information by Applicant / Driver
I-9 Employment Eligibility Verification
W-9 Request for Taxpayer Identification Number and Certification
